- 3Submit your opt-out request by completing and submitting the form.

- 1Visit the Club Pilates website (clubpilates.com).
- 2Open the cookie consent or cookie management tool, typically accessible via a banner or link in the website footer.
- 3Click 'Reject All Cookies' to block all non-essential cookies, or choose 'Customize' to selectively disable advertising and targeting cookies.
- 4Alternatively, adjust your browser settings to block or delete cookies from clubpilates.com.
Data access portability

- 2Select 'Club Pilates' as the brand and indicate you are making an access or data portability request.
- 3Provide enough identifying information so Club Pilates can verify your identity.
- 4Submit the request. You may make this type of request up to twice in a 12-month period.

- 2Fill out and submit the consumer request form, selecting 'Club Pilates' as the relevant brand.
- 3Provide sufficient information to verify your identity (e.g., name, email address associated with your account).
- 4Submit the form. Club Pilates will endeavor to respond within 45 days of receipt.
